I used to have lots of scattered names and numbers all over the place.  This
is an attempt to bring that information into one place.  It is an extremely
simple address database, consisting of only the following:

  id 
  name 
  phone 
  state
  city
  zip
  street
  misc 
  email

This is not full-featured, nor should it be.  Supports create, drop, edit, 
search.

Most (okay, all) of the code is hardwired and written very poorly.  However, it
does *exactly* what I wanted it to do.  If you choose to modify it, please
feel free to do so, and submit the changes back to me.
